A near-miss is defined as a sequence of events and/or conditions that could have resulted in loss. This loss was prevented only by a fortuitous break in the chain of events and/or conditions. The potential loss could be human injury, environmental damage, or negative business impact. Reporting of near-misses plays an important role in learning from istakes, and preventing accidents and their serious consequences. All ships plying in Singapore waters and the Singapore Strait are encouraged to report to the Maritime and Port Authority of Singapore if near-misses are encountered and/or observed. These reports are confidential and not intended to apportion blame or to determine liability, but to assist in improving the safety of navigation.
