Guidelines and pre-arrival requirements

The Palawan Station of the Bureau of Quarantine is requiring the concerned Ship Agents/Agencies of their respective vessels to furnish our office with the following Pre-Arrival Documents, at least 48 hours before arrival, for our review:

  1. Voyage Memo or Port of Call List
  2. Crew List, including Sign On Date and Place of each Crew/Officer
  3. Temperature Log of Crew/Officers
  4. Health and History of Travel/Exposure Declaration

The Master must report immediately any occurence of flu-like symptoms (e.g. fever, cough, colds, body weakness, and/or difficulty of breathing) for guidance by the Bureau of Quarantine on implementation of infection prevention and control (IPC) protocols to minimize spread of infection on board.

The Ship Agents/Agencies are likewise requested to provide the ships’ crew and passengers copies of the Health Declaration Card for them to accomplish prior to arrival.

Therefore, aside from the usual documents required by the Bureau of Quarantine, the following additional documents will be collected during Quarantine boarding formalities:
1. Health Declaration Cards (fully accomplished and signed by the crew/officer),
2. Temperature Log (latest)
