Compulsory Maritime Health Declaration

Singapore's National Environment Agency (Port Health Office) requires all arriving vessels that have been to the Ebola affected countries (i.e. Guinea, Liberia, Sierra Leone, Nigeria, Democratic Republic of Congo and Senegal) in the past 21 days to submit the Maritime Declaration of Health Form to the Port Health Office. As per the Maritime and Port Authority of Singapore Port Marine Circular No.12 of 2014 issued today (30 September 2014), this is part of the measures to prevent the introduction of Ebola Virus Disease (EVD) into Singapore. The procedure, which comes into immediate effect, states: The Maritime Declaration of Health Form is to be submitted regardless of whether there are sick passengers or crew on board. It should be submitted to the Port Health Office via: a) Fax: 622285643 b) Email: [email protected] For information about operations in Singapore contact GAC Singapore at [email protected]
